How it works

Pick the form, then fill in the lines that apply to you. Type each amount the way it appears on your pay stub or bill and choose how often it comes: a month, a week, every 2 weeks, twice a month, a year, an hour or a day. The page turns it into a monthly amount with the method the form's instructions give, adds every subtotal and total in the form's own line numbers, and works out your surplus or deficit. The answer sheet lists every line in order, with the working, so you can copy it onto the official form and check each figure.

Example: paid $1,500.00 every two weeks, line 1 is $1,500.00 × 26 ÷ 12 = $3,250.00. A $1,200 car insurance bill twice a year is $1,200.00 × 2 ÷ 12 = $200.00. Open a filled-in short form example or a long form example.

This page does arithmetic only. It is not legal advice and it is not the court form. You sign the affidavit under penalty of perjury, so check every amount against your pay stubs, bills and statements before you copy it.

The official forms

Download and file the form itself from the Florida courts: Form 12.902(b), short form, revised 10/21 and Form 12.902(c), long form, revised 06/25. Each PDF starts with the instructions this page follows. Where the long form is unclear (the line 39 subtotal and line 95), the answer sheet says what this page did and why. If someone who is not a lawyer helps you fill out the form, the instructions say that person must first give you Form 12.900(a), Disclosure from Nonlawyer, and put their name, address and phone number at the end of the form.

Questions people ask

Do I use the short form or the long form?

The instructions say: if your individual gross income is under $50,000 a year, use Form 12.902(b), the short form. At $50,000 or more, use Form 12.902(c), the long form. Either party can also ask the other to fill out the long form by serving the standard family law interrogatories.

How do I change weekly or biweekly pay to monthly?

The form's instructions say: weekly times 52, divided by 12. Every two weeks times 26, divided by 12. Twice a month times 2. Hourly times hours a week, times 52, divided by 12. Expenses are converted the same way. So $500 a week is $2,166.67 a month, not $2,000.

Should I round to whole dollars?

The instructions don't say. The official fillable PDF shows amounts with cents, so this page keeps cents and rounds each converted amount to the nearest cent. Totals add the rounded line amounts, so the sheet adds up exactly as you copy it.

Can the financial affidavit be waived?

The instructions list three cases where it is not needed: a simplified dissolution under rule 12.105 where both parties waived it; no minor children, no support issues and a filed written settlement agreement covering all financial issues; or a court with no jurisdiction over financial issues.

What if some expenses are estimates?

The form asks you to write “estimate” next to each amount that is estimated, for example a rent you expect to pay after you move. Tick Estimate on that amount here and the answer sheet marks the line.

Does this work out child support?

No. The affidavit only asks you to tick whether a Child Support Guidelines Worksheet, Form 12.902(e), is or will be filed. That worksheet must be filed at or before a hearing to set or change child support.

Who can see what I type?

Nobody else. It is saved only in this browser on this device, as you type, and nothing is uploaded. Download a backup file to move it to another computer. On a shared computer, use Delete everything when you are done.
